Alternative Home Health Care provides compassionate home care services. Skilled Nursing

Our licensed nurses oversee our patients' comprehensive care and ongoing medical needs. This can include skills such as assessments and coordination of care, managing a patient's medication, or extensive treatments and therapies following an illness, surgery, or other health event. Our nursing team will work with you and other caregivers to provide a personalized treatment plan to maximize our patients' comfort and help meet their health and wellness goals.

Wound and Ostomy Care

Our certified medical staff provides professional wound care assessment and managed treatment services. We use a multidisciplinary approach in the treatment of both acute and chronic wounds, ranging from using appropriate dressings, wound cleansing, and ongoing management and healing plans. We have wound and ostomy-certified clinicians.

IV Therapy

With our IV therapy service, you can receive intravenous medications and phlebotomy services in the comfort of your home. Your nurse will arrive promptly to provide your prescribed treatment and take blood for diagnostic testing. We partner with healthcare providers, specialists, and labs to ensure the highest level of care. We have IV-certified clinicians on staff.

Medication and Disease Management

Alternative Home Health Care's nursing and medical staff is experienced in assisting patients with various mental health and medical issues. This can include everything from overseeing a medication management program by our licensed nursing staff to long-term disease management and care. Medication management via a locked box is available for safety if necessary. We also provide pain management programs managed by our nursing and rehabilitation team.

Patient and Caregiver Education

Our staff is here to ensure our patients and their families feel confident about their level of home care, not only when our nursing and medical team is at your home but also when we aren't. We believe in providing the knowledge and resources to ensure your loved one receives the proper care and specialized services.

Social Worker

Alternative Home Health Care conducts social work, including assistance with transportation, offering community resources, and helping with ongoing support. We coordinate cases with managed care insurance and aid with discharge planning from the facility to the community.

Physical Therapy

All our home care services are designed to meet and exceed our patient's needs, whether short-term recovery or long-term physical therapy and rehabilitation. Our physical therapists specialize in fall prevention programs, ultrasound, and electrical stimulation therapy (E-STIM).

Occupational Therapy

Occupational therapy helps patients recover, develop, and strengthen the skills and movements needed for daily activities, from self-care routines to motor skills and planning.

Speech Therapy

Our Speech Language Pathologists partner with patients and their families to create a treatment plan designed to help with language ability and alternative communication and address related issues, such as swallowing dysfunction. We implement a variety of proven therapies to assist in the treatment of our patients.

Home Health Aide

Alternative Home Health Care provides our patients with services that can help bolster independence and comfort, which is vital. Our home health aides offer patients various activities of daily living (ADL) and instrumental activities of daily living (IADL) services, including help with light housekeeping, meal preparation, and assistance with self-care routines.

Maternal Child Health (MCH)

Alternative Home Health Care routinely provides in-home well-baby visits and welfare checks of newborns. This can be especially important if the mother and child are discharged from the hospital early, there are any complications, or the Department of Children and Families (DCF) is involved.

Specialty Programs

We partner with many specialty programs such as, Elder Services, Acquired and Traumatic Brain Injury Program, and Veterans Program to provide rehabilitation and custodial care for their members. This care includes personal care assistance and light housekeeping provided by our paraprofessional staff, who are supervised by our licensed staff. Through our sister company, we can coordinate non-medical transportation services to get patients to and from doctors and other health-related appointments.
